How to Register for a Berg Adventures Trip
Reservations
The best way to reserve a place on one of our trips or courses is to call our office, confirm that there is space available on your chosen trip and secure your reservation with a credit card deposit (AMEX, VISA, or MC). You may also fax or mail a completed application form with a check, money order or credit card number.
There are four forms which need to be completed before you travel with us. All forms are available for you to download and print, or we will be happy to mail or fax them to you. Just call us or send e-mail. Adobe Acrobat is required in order to download and print these forms. Most computers already have the Adobe Acrobat reader installed, but you can also get the reader for free at the Adobe link shown below.
The complete application packet for a BAI trip is in four parts
- Application – complete and submit with deposit
- Terms and Conditions – read carefully, sign and return
- Waiver – read carefully, sign and return
- Medical Form – complete fully, sign and return. We encourage you to use additional pages and provide details about your health and medical history.
A copy of our Trip Evaluation Form is also provided below. This form will be mailed to you after you return home from your adventure. But in case you lose it or want to download a copy while still traveling it is available here.
